Application form and the detailed information viz. qualification, experience, age limit etc can be obtained from the Committee Branch, Kairon Block, PGIMER, Chandigarh personally or on written request accompanied by a self addressed stamped (Rs. 10/-) envelope (27×12 cm) along with Demand Draft worth Rs. 525/- (Rs. 275/- for SC/ST candidates) in favour of the Director, PGIMER, Chandigarh payable at State Bank of India, Sector-12 (Code No. 1524) as non refundable fee.
A demand draft for Rs.500/-(Rs. 250/- for SC/ST candidates) should be attached with the application downloaded from the website towards non-refundable application fee.
